What is lung cancer?

Lung cancer arises when cells in the lungs grow abnormally and out of control. The tumor tissue impairs breathing and, in later stages, can spread to lymph nodes and other organs.

What are the types of lung cancer?

Lung cancer is divided into two main groups by cell structure:

Non-small cell lung cancer (NSCLC)

About 85% of cases. Includes adenocarcinoma, squamous cell, and large cell carcinoma subtypes.

Small cell lung cancer (SCLC)

About 15% of cases, largely linked to smoking. Grows fast and spreads early.

Causes and risk factors

The most important cause is smoking and tobacco use, but it can also occur in never-smokers:

  • Exposure to radon, asbestos, arsenic, and industrial chemicals
  • Air pollution
  • Family history and genetic predisposition
  • Chronic lung diseases such as COPD

What are the symptoms of lung cancer?

Often no symptoms early on; complaints usually appear as the disease advances:

  • A persistent cough that worsens over time
  • Blood or rust-colored sputum
  • Shortness of breath and wheezing
  • Chest pain that worsens with breathing or coughing
  • Hoarseness
  • Unexplained weight loss and loss of appetite
  • Constant fatigue
  • Recurrent pneumonia or bronchitis

What are the stages of lung cancer?

Non-small cell lung cancer is generally assessed in four stages:

Stage 1

The tumor is small and confined to the lung, with no lymph node spread. This is the stage with the highest treatment success.

Stage 2

The tumor has grown or begun to reach nearby lymph nodes.

Stage 3

The cancer has spread to surrounding tissue and regional lymph nodes.

Stage 4

The cancer has spread to distant organs such as bone, liver, or brain (metastasis).

Small cell lung cancer is usually described as "limited" or "extensive" stage.

How is lung cancer diagnosed?

Diagnosis is confirmed with imaging and tissue sampling after history and exam:

  • Bronchoscopy to inspect the airways and take samples
  • Needle biopsy with pathological examination
  • Molecular and genetic tests (EGFR, ALK, PD-L1) for targeted therapy
  • Sputum cytology

Imaging methods in lung cancer

Main methods used for diagnosis and staging:

  • Chest X-ray
  • Low-dose CT — screening and diagnosis
  • PET-CT to assess spread
  • Brain MRI (metastasis screening)

Lung cancer treatment methods

Surgery

Removal of the tumor/lobe in early stage; VATS and robotic minimally invasive techniques in suitable cases.

Radiotherapy

Targeted radiation using advanced techniques such as SBRT and IMRT.

Chemotherapy

Targeted therapy

Smart drugs for tumors with mutations such as EGFR and ALK.

Immunotherapy

Prognosis and survival rates

Survival in lung cancer depends heavily on stage. In early-stage disease confined to the lung, 5-year survival can reach about 60%, while it drops significantly with distant metastasis.

In recent years, targeted therapies and immunotherapy have improved survival and quality of life even in advanced disease. Early diagnosis is the single biggest factor in prognosis.
